PAYMENT OPTIONS
AUTO PAY PLAN
Employees will take full responsibility of payment, enrolling with their own Credit Card or Checking Account information. Payment will be deducted every 1st of the month on their personal account. All employees must sign Membership Forms with HR Department. Benefits: Employees take care of their own payments.
PAYROLL DEDUCTION PLAN
Company is invoiced on a monthly basis for employee dues. Company deducts the amount from employee payroll as convenient. Benefits: Payroll deduct memberships are economical and easy to manage; Employees like it because it is convenient; High level of employee participation: typically, 20% to 40%.
TERMS & CONDITIONS
- Corporate memberships are exclusive for Employees of the company.
- Only immediate family members of Employee will receive discount: Spouse will receive 10%off. Children 17 and under will be $40 + tax. Children 18 and over will receive 10%off (must belong to same household and attending school. Must show school ID)
- Employers must inform us of any changes in company that may affect membership (such as hiring or release of employees).
- Minimum of 10 Employees is required in order to qualify for the Corporate Rate.
- Employees must use the Payment Plan set up by the Employer.
- Should memberships fall below the minimum requirement of 10 members for a three-month period, the Corporate Rate could be subject to change.
- Corporate Rates are subject to change with 30-day written notice.
- All Employees must fill out Membership Agreement with their HR Department or HAHF.
- No initiation fee, no cancellation fee, no late fee.
- Six-month commitment (unless Employee is terminated of work).
